HOW TO APPLY FOR MEMBERSHIP
Any individual 18 years old & above can fill out an application form at the Club Ultima Membership Office
Present valid ID (Company ID, SSS, Driver’s License, or Club Ultima membership club)
Prepare payment. For check payments, it should be made payable to Club Ultima Philippines, Inc.

- 50% discount for USSC Membership
- Operating Hours: 11 am - 8 pm daily
